<p>Recently, there has been lots of information in the press about the use of <a href="http://itunes.apple.com/us/podcast/plastic-surgery-podcast/id377200019" target="_blank">polyurethane implants</a> for breast enlargement, and there has been resurgence in their use.<span id="more-1401"></span></p>
<p>The main advantages of polyurethane implant over conventional textured implants are that they:</p>
<p>1. Reduce the rate of capsular contracture;</p>
<p>2. Reduce the risk of implant descent.</p>
<p>The polyurethane-coated implant adheres to your natural body skin internally, and because of this adherence, the rate of capsular contracture is reported to be in the region of 1 in 100 rather than 6% to 10% with conventional textured silicone implants.</p>
<p><a href="http://itunes.apple.com/us/podcast/plastic-surgery-podcast/id377200019">Polyurethane implants</a> have been used for many years in Brazil and Europe, and are often used for people who have developed capsules with traditional implants.</p>
<p>The other main advantage of the polyurethane implants is that they adhere internally to your breasts and unlike traditional silicone implants do not tend to be affected so much by gravity.</p>
<p>Gravity effects to the traditional implants can cause lowering of the implants and bottoming out in which the lower portion of the breasts is stretched unnaturally.</p>

<p>The answer to this is yes, and this is a combination of procedures I have performed relatively often.</p>
<p>The breast enlargement itself often does push the nipples out slightly which improves the nipple inversion to a mild extent.</p>
<p>If, however, the nipples are very inverted (grade 2 or 3), an additional nipple inversion correction procedure can be performed at the time of surgery.</p>
<p>It is an extremely effective procedure which addresses the changes of the volume and shape of the breast together with the nipple.</p>
<p>The results are very good in that all the elements are corrected leading to natural-appearing breasts with normal nipples on them.</p>

<p style="text-align: justify;">People develop rippling in the upper part of the<a href="http://www.aurora-clinics.co.uk/treatments_chest.html"> breast</a> because there is insufficient soft tissue padding over the implant. This allows the outline of the implant to show through the skin.</p>
<p style="text-align: justify;">Rippling can be avoided by adding an extra layer of muscle between the implant and the breast, and it is performed when the implant is right behind the <a href="http://www.aurora-clinics.co.uk/breast_over-or-under.html">breast muscle</a>.</p>
<p style="text-align: justify;">If you have developed rippling, the option to treat this include exchanging the implant for a firmer (more cohesive) implant which is less prone to rippling and it is less likely to show.</p>
<p style="text-align: justify;">The other option is to place the implant behind the muscle of the <a href="http://www.aurora-clinics.co.uk/BreastAugVideos1.html">breast</a> which again conceals the implant.</p>

<p>Saline implants are essentially composed of a thin silicone shell and filled up with salty water (saline).</p>
<p>The advantage of these <a href="http://www.aurora-clinics.co.uk/treatments_chest.html">implants</a> is that they are not silicone and because they are collapsable they can be inflated inside the body and therefore can be inserted through very small holes. This is the type of implant that is inserted through the tummy button as it would not be possible to insert a much larger uncollapseable <a href="http://www.aurora-clinics.co.uk/BreastAugVideos1.html">silicone implants</a> through this hole.<span id="more-832"></span></p>
<p>The disadvantage of saline implants is that they are prone to leakage and feel much more unnatural in the body being firmer and more prone to wrinkling.</p>
<p>On balance, if having implants inserted through a very small incision is important to you, saline implants are a possibility.</p>
<p>If however natural feel and more long term stability in the implant are more important, I would advise you to opt for the latest fourth generation cohesive silicone gel implants.</p>
