Breast Reduction Combined With Tummy Tuck

Hello. My name is Adrian Richards. I’m a plastic and cosmetic surgeon and I’m the Surgical Director of Aurora Clinics based here in the United Kingdom. In this video, I’m going to be taking you through an operation I’ve just finished on a lady who had a joint tummy tuck and breast reduction procedure. This lady’s lost a large amount of weight, and I’m reducing a significant amount of tissue from both of these areas. So I’ll take you through each stage of the operation, and I hope you find it interesting.

Here’s our patient before the operation and this is with my markings. You can see I’m going to lift the nipple up to that new position and remove that tissue between my markings on the tummy.

This is in the operation. I’m going to start with the breast tissue, and as you can see what I’m aiming to do is remove this area here, all this shaded area, and bring the skin together and lift the nipple up to that new position.

The first step is really to mark out the boundaries of the new nipple. I’m going to reduce the areola. That’s a nipple marker we use to do that to make it nice and round. I just tattoo the four quadrants of the nipple to help me orientate it.

This is the tissue I’m going to remove. You can see all this tissue’s going to be removed and the skin brought together there and the nipple brought up there.

So you see this is actually after I’ve removed the tissue. So that’s the tissue removed.

Here I’m just showing you again, remove that from above and outside the nipple, and that’s the lower bit which I’m removing in this case. You can see what I’m left with is a bridge to the nipple which will preserve the blood supply and the nerve supply, and the nipple’s now sewn into its normal position and the skin closed with those absorbable stitches which I’ve shown you before.

The next stage is just really to put some glue on the area just as a final closure to hold the skin together. My dressings will be with Micropore tape, which is brown tape, and the patient can wash and shower in that.

You can see here this is the nipple, and you can see it’s got a good blood supply. Can you see as I push? Nice and pink and the blood returns coming back to it nicely. This is the bra inserted there, and this is me with the breast tissue which I’ve removed which will be sent for analysis.

Now I’m moving on to the tummy. This is the tissue I’m going to remove from the tummy area, really from the Caesarean scar to the tummy button.

This is me just marking the tummy button and me cutting down onto it because I’m going to preserve this as a stalk with its blood supply and as you’ll see in a minute, bring it through the skin. You can see here I’m cutting down on either side of the tummy button there and just bring it up a big with my scissors and you can see the stalk.

Here is just cutting through the lower part of the skin there. Then I just go slowly up, develop that and lift up the tissue from the underlying muscle wall of the tummy. Then cut down the middle to the tummy button, and this is the tissue that’s going to be removed. You can see here’s the tummy button from the inside on its stalk.

This is me carefully marking the area I’m going to remove. This is me just removing the excess skin in that region, cutting around the tummy button that’s going to be the new hole for the tummy button that the stalk’s going to come through. And here, removing the skin.

You can see I’ve got approximately 23 centimetres of skin I’m going to remove in this case. Here I am just removing that skin and here is the skin that was removed. I do it in two halves, one from either side, so you can see the skin there which is holding up.

Here’s the tummy all finished with the Micropore tape on and a nice dressing over the tummy button.

Here’s the weight we’ve removed. As you can see, quite a lot from the tummy.
