Breast Implant Surgery Exchange
Hello. My name is Adrian Richards. I’m a plastic and cosmetic surgeon, and I’m the Surgical Director of Aurora Clinics based here in the United Kingdom. In this video I’m going to be taking you through an operation I recently performed on a lady who had a bilateral breast augmentation procedure performed three years ago. She’s very happy with the breast augmentation, in which the implants were placed in front of the muscle, but has had a child a year ago and with subsequent enlarging of the bust feels now that the skin is a little bit loose around the implants. So I’m going to increase the implants to 520 cc implants from 325 cc but keeping the implants in the same plane, in front of the muscle.
Here we can see the patient in the operation, and I’m just showing you I’d like to move the implant a bit more centrally and that’s the scar I’m going to remove.
Here I am just removing the old scar. I tend to remove the old scar rather than make another incision, as this will result in one scar rather than two scars which is much neater. You can see me just gently removing the scar.
The next stage is to go down to the implant, and you can see that’s the implant just under my electrocautery there. I’m just going to cut down onto the implant. You can see the implant coming into sight there.
Then the next stage is really just to spread the area and remove the implant. In this case, there is some fluid around the implant as you can see. The implant is in good condition and is a Silimed 325 cc implant.
The next stage is just to free up the capsule to allow for a bigger implant. You can see I’ve done that here.
On the left is the sizer I’m going to use, which is inflatable. On the right the old implant. The sizer is a 500 cc size, and I’m going to increase this slowly with a syringe here to gauge the optimum fill for this lady’s breast pocket. With pregnancy, the skin envelope has gotten larger so she’s going to need a larger volume implant than the 325 cc implant that she had in previously. Then you can see we’re just gently injecting saline 50 ccs at a time into my temporary inflatable implant, all the time just checking on the breast and seeing when we get good fill particularly immediately to make sure that the cleavage is in a good position. This gives me really fine control over the size of the resultant implant.
Here I am really quite happy with the sizer now. Larger on the right, better cleavage. I’m going to remove the temporary implant. This is me putting some antibiotic solution in the pocket around the implant. The next stage is to put some disinfectant around the skin because this is going to touch the implant as I insert it. I’ll then put on a fresh pair of gloves, open the implant, and then insert it in this fashion there.
This is with the implant inserted, and you can see I’m just double checking everything looks good as it does in this case.
I then sew up the area in three layers. This is the deep layer onto the capsule around the implant. This is with the completed layer, you can see fully secure. I’m going to do two more layers on the skin to close. So I’ll have a three layer closure on this area.
This is after the second layer of stitches, and this is after the third layer. I’m actually applying a layer of Dermabond tissue glue just to finally seal the area, and you can see I’m holding the stitches on either end there.
Here you can see this is the completed right side. You can see it’s larger than the left, much better bust, much better shape and size.
