Unilateral Breast Implant Exchange Operation
Hello. My name is Adrian Richards. I’m a plastic and cosmetic surgeon and the Surgical Director of Aurora Clinics. In this short video, I am going to be taking you through an operation in which I exchanged a left side breast implant. Our patient today has had textured shaped implants inserted some years before, but she’s noticed a change in the left side. The shape and the consistency of the left side have changed in the last couple of years. So, I’m going to explore the left side via the old incision and we’ll see in the video the state of the implant.
So this is the beginning of the operation, and you can see I’m just making an incision exactly on the old scar here. I’ve marked it previously, so I’m not going to give this lady another scar. Just going to have the same scar.
Now, when I went down to the capsule, there’s quite a lot of fluid around this implant, which is quite characteristic of older implants which have been in for some years. Interesting, when I removed the implant, it had rotated 90 degrees anti-clockwise. Can you see that’s the position it should be in, and that’s the position it was in? So, the white stripe should be at the bottom at 3 o’clock, and it had rotated up to the 3 o’clock position.
So, I cleaned everything out. Now I’m putting some antibiotics in the cavity now. Now I’m just going to insert a brand new implant, exactly the same size and shape of implant now as the old one. But can you see the old implant is on the left, new implant is on the right? You can see that the right newer implant is much fuller and has got much less rippling in its upper portion, and I think this is really because of the improved fill quality of implants recently that they tend to be slightly more filled and not so prone to wrinkling as the older implants.
So, I’m going to insert that implant in exactly the same position, as you can see the difference again between the two sides. Here is the implant inserted. You can see the white line which shows me that it’s absolutely correctly orientated in the right position. Just showing you, again, there’s the white line showing you that the shaped implant is orientated. Obviously, it’s very important particularly with shaped implants that they are orientated absolutely correctly and do not rotate, as this will change the shape of the bust.
Here we are at the end of the procedure with the implant on the left exchanged for the brand new cohesive gel shaped implant.
