Our patient today’s got breast implants. They’re in front of the muscle. She’s got quite a long incision there from a previous surgery. I don’t know if you can see that mark there. That’s where the drain was inserted. I don’t use drains now, but that’s where the drain was inserted. You never quite know what you’re going to find. The ultrasound has shown that the implants are folded. I’m going to make an incision along that old incision, and I’ll show you what we find inside.

I’m just doing an incision carefully along the dotted line there. So I’m trying to keep exactly to the dotted line.

I’ve cut down into the implant. I don’t know whether you can see, but it’s quite wrinkly. Can you see there are wrinkles in the implant there?

So, the ultrasound was right. The implant is wrinkly. It looks like quite a thin walled, textured implant, and it is quite wrinkly, as we suspected. So I’m just going to remove it now, and then we’re going see what size it is.

This is quite a flat implant, textured, but very wrinkly. Can you see how wrinkly it is? It’s a 225cc implant. Quite and old fashioned implant.

This is an inflatable implant I’m going to insert now. I haven’t inflated it, but I will inflate it with this syringe and then, essentially, just fold it up, like I’ve shown in lots of other videos, in the cavity and then blow it up.

So, the previous implant was a 225cc, and our patient today wants to increase that probably to 460. So, that’s 100, 150. It’s going up little by little. I’ll show you when we get there.

I’m just putting some antibiotics now into the cavity. We’ve worked out from our sizer that the 450cc implant is the optimum implant for her. It’s much bigger and much more domed. I’ve also released inside. I performed what’s called a capsulotomy, releasing the capsule] inside, and I’m just going to insert the implant. I’ve got brown betadine, which is antiseptic, on the area that’s connected with the implant.

We can see I’ve now put the 460cc implant here. We’ve got a 250 on this side. I don’t know if you can see the difference in the volume. We’ve got better cleavage on the right side here, a more fuller appearance. We’ve got a slightly more high profile implant, and I think we’re going to get a better breast shape.

That’s the old implant. Can you see all the wrinkles in it? It’s very under-filled. I don’t know if you can see that. The characteristic of older implants is that they’re more wrinkly. Our new one here is filled with much more cohesive gel and is much more solid. I think you can see the difference. Although there’s not much difference in diameter, the newer implant is higher and much less prone to wrinkling like the older one.

Here we are at the end of the procedure. I think we’ve got really quite good breast symmetry, and just moving around we can see we’ve got a quite nice contour to the chest by its size. So removing that old, wrinkled, thin walled implant and replacing it with a high profile, more modern implant I think has given a really good result in this case.
