Hello, my name’s Adrian Richards, and I’m a plastic surgeon and the Clinical Director of Aurora Clinics. In this video I’m going to be showing you a PIP replacement video, in which we remove ultra-high 335cc implants which were put in, in 2006.
Scans have shown that the implants are probably intact, but they’ve got quite a lot of rippling and palpability. So I’m going to exchange them for Nagor implants, and our patient would like slightly better cleavage, so I’m going to insert slightly wider implants to help with this.
I’ve just removed this implant, which is a 335cc PIP implant. It was badly placed. It was sort of rotated and was almost lying upside down. And we can see here, this is the silicone gel bleed that we get from the PIP implants, much more than normal implants. Can you see the stringy silicone? And also get deflation at the top of the implant. So you can see there that the top of the implant’s really completely deflated there. So that’s why our patient’s been noticing loss of volume on the top, and just really because there’s gel bleed.
So the next stage is to clean everything out and put in a sizer, which is a temporary implant. This is a temporary inflatable implant, which I can blow up. Can you see where the fluid’s going in there? So what I’m going to do is put that in, and then blow it up, and you can see it’s much wider. So blow it up, see which is the optimum volume for our patient today. The first stage is just to fold it up evenly and put it in the cavity. Then make sure it’s sitting correctly, and then you blow it up.
That’s 50 ml, and now this is 100 ml, 150, 300, and that way you can see the cleavage is improving. There’s 300, 350, 400cc’s. 460 [inaudible 2:40] really good actually. So that’s 400, 460. So that way you can see that the cleavage is [inaudible 2:54]. I think that will be good, 460, on our lady today. Great. So we go to the 460. So we’re going to get fuller and a bit wider.
Can you see there’s more silicone gel bleed on this side, and the implant there is deflated at the top. So this is pretty classic, deflated ultra-high implant with silicone gel bleed. That certainly is a characteristic we’re finding from PIP. So now, I’m going to clean everything out, take a specimen of the capsule, and then replace it with a Nagor implant.
So this is just the end of the operation. You can see we’ve got a better cleavage, fuller appearance to the breasts, and in a minute I’ll just show you the implants.
So I just thought I’d show you the implants now they’re out. This is the right implant, if you have a look down. It’s a 335cc ultra-high implant, and the lot number is 906. And on the left the lot number is 6306. The implants had quite a lot of gel bleed, as you saw. When they are exposed to the air, the gel bleed decreases. Basically, the implant dries, but you can see that the top of the implant here is deflated and the silicone tends to go to the bottom. This is a very characteristic appearance of a PIP implant, basically losing its fullness in the upper part. Here’s another one. If I put this bit as it would be in a person, there’s probably slightly less gel bleed, actually, in this one, slightly less deflation, but still some deflation there.
So overall, I think good to remove the implants in this lady. I think they were too narrow for her, because she had quite broad shoulders. So we’ve used broader implants. They are going to give better cleavage, and hopefully a better result, and also, obviously, having a removal of the PIP implants and their gel bleed.
