PIP breast implants are being removed without replacement by plastic surgeon Adrian Richards at Aurora clinics in Milton Keynes. The patient has had the PIP implants for quite a while, but she decided to have them removed as she was concerned about her health being affected if breast implants were to rupture. After having the implants removed, she had enough breast tissue left to keep a good appearance of her breasts and not lose breast volume completely. The PIP implants were intact, but with significant gel bleed as it can be seen in this video.

So, hello, my name is Adrian Richards, and in this video I’m going to be showing you a video of a patient who’s decided to have her PIP implants removed. She’s had children, and now has decided she doesn’t want them and I’m going to be taking the implants out in this operation.

Our patient today wants removal of her breast implants. This is her old scar here, which is quite wide. So I’m going to first of all remove the scar, remove her PIP implants, and I’ll show you what the state the PIP implants are in.

This is the scar, which I’ve just taken away here, and back to fresh tissue on either side so when I stitch that, we’ll have better scarring.

So there’s quite a lot of silicone gel bleed around this one. Don’t know whether you can see that. We’re sucking it out now. Silicone gel bleeds often towards the side because when the patient lies down, that’s where it would tend to fall down to.

I’m just going to remove the implant now. Here we go. So again, probably slightly less silicone gel bleed on this side, but there’s still . . . I don’t know whether you can see those globules of silicone on the outside there. And again it’s a PIP. Slightly strange numbering on this one. These, well, the earlier implants had a different back plate. It says “PIP 230cc France,” and we’ll just read those. It’s a bit difficult to see them. Anyway, there’s quite a lot of gel bleed as we expect on the implants, but at least the implants are intact.

So, I’ve just removed the implants, and I don’t know whether you can see there was fluid on the outside. Can you see? Actually globules of fluid on the outside. And this is all gel bleed again we’re seeing. A bit more than usual I would say with this implant, very slippery and slimy, with a lot of gel bleed. And it’s a 230cc. Slightly different writing. They changed the writing on the back of the PIP implants. There’s slightly bigger writing on here. Looks like the earlier implants. You can see, again, a lot of gel bleed.

So we’re just going to clean everything out now and wash out the cavity, take a specimen of the capsule for histology.

So this is just the end of the operation. As you see, our patient’s still got quite a lot of breast tissue on both sides. That’s the incision all stitched up, and the implants I’ll show you in a second.

Now I’d just like to show you the implants close up. As you can see, when they were removed, they had quite a lot of gel bleed. If we just go down here and have a look, this is the right one. So it was PIP batch number 13000-188, 230cc. As you can see, it’s actually got quite a different back plate from the normal PIPs. This is quite an early PIP, 230cc, and the etching on them is different in these early ones. It’s got “France” written across here. So this back plate is different on these early ones, which I think was put in, in 2002. So the early ones were slightly different. But the implant quality was the same. As you can see, the gel bleed’s gone there.

If you go on to the left one, here very similar appearance. The same batch. Instead of 188, it was 114. Again, same sort of etching on the back of the PIP implants. It’s much smoother, the etching on these more recent ones. But again intact, but a very early PIP implant, about 2002. So one of the first silicone implants they made, when they started making silicone implants in France in the early part of 2000s. But intact, but with gel bleed, so good to get rid of them.
