PIP Breast Implant Replacement and Implant Upsize

So our patient today has got PIP implants. She would like them slightly larger. In particular, she would like better cleavage in this area here. So what I’m going to do is free up this whole area here to give her better cleavage on both sides here. So the implant can go in more that side and put a larger implant in.
The first thing is to remove the old scar. It’s quite a good scar actually, but I’m just going to remove it anyway because you get better scarring if you just remove the old scar. I’ll remove that, have a look at the implant, free up here, and then put my sizer in, which is a temporary implant to see what the best size for her is.
So we’ve got an intact PIP implant. We’ll record the serial number. Have a look at the size. You’ve just got to catch the light right to see the size, 390 cc, PIP 390. Yeah, that’s great, 390 cc, lot number 503367. So you’ve got some gel bleed. Can you see that? Otherwise, it’s intact condition. So we’ll just store that lot number. Now, I’m going to free it up internally.
So I just removed this one. This is a 350 PIP. Again, lots of gel bleed. You can only see it on the outside, and again this implant just doesn’t feel great. I don’t know if you can feel there’s a big fold in it there. That’s where they tend to rupture. So, although not ruptured, again I think it wouldn’t take much to get them to be ruptured, a few years probably and they would.
So I’m just going to clean everything out, take a specimen of the capsule, free up the capsule again, and go for a larger implant.
Here we are at the end of the operation. I don’t know whether you can see we’ve filled out to there and on this side also filled out to the line. So, before we were very much here, and now we’ve got a much better cleavage on both sides.
Here we are at the end of the operation. I’m just going to show the implants, which, of course, were intact. So this is the right one here, so an intact PIP. We’ve recorded the serial number, but I think it’s 503367. Now, it’s intact, but I don’t know whether you can see. It’s not very good quality, and they tend to deflate like that, and that’s the area that tends to rip. If I wanted to, I’m sure I could rip that. I don’t really want to. So, can you see how the implant is really deflated there because the gel sort of leaches out.
Then, on the other side, there’s a similar sort of appearance. You get a very firm rim. So most patients can feel the rim of their implants like that. So, as the silicone gel leaches out, they can feel the rim on the sides. It’s good to have them removed, I think. Now we’ve good Allergan implants and it’s a better fit.
