PIP Breast Implants Exchanged After Gel Bleed and Deflation

Hello. My name’s Adrian Richards. I’m a plastic surgeon and the Surgical Director of Aurora Clinics. In this case, I’m going to be doing an exchange of implants, PIP implants, in a lady who had two 265 cc implants inserted back in 2006 and has elected to have the implants replaced today. She has noticed some deflation on the right side, which she feels lost volume on the upper part and changed shape slightly. So we’ll be having a good look at that and using a larger implant to give more volume to her bust.
Our first stage, I’m going to remove the old scar, which is actually quite a good scar. Then I’ll remove the implant, put an intraoperative sizer in, which is an inflatable implant, inflate it to see what size best fits our lady. But I think what we’re looking for is really to get better cleavage here, better fullness here, and better fullness on the top. I think the implants were a bit small and narrow for her.
You can see this implant on the right is deflated in the top. Can you see that? It’s got silicone gel bleed here. Can you see that stringy silicone gel bleed? It’s quite a high profile implant, quite fine texturing, but this is the point where they rupture on the side. As we get gel bleed, it tends to deflate at the top. This I would say is probably a slightly better than average condition PIP implant. I’ll just remove that. This, again, has a significant amount of lack of volume from the upper portions and waviness of the implant.
I’ve just removed this implant, which deflated quite a lot. I don’t know whether you can see that big dimple in the middle. It’s got gel bleed on it, and you can see that when it stood up, that we’ve lost the volume in the top here because of the gel bleed. I’m going to clean everything out, enlarge the pocket a little bit and put a slightly larger implant in.
This is the appearance at the end of the operation. I’ve increased from 265 to 520 cc implants, so nearly the double the size, but I think it fills out her medial area here much better and down inferiorly and it gives a much better breast shape. Even though they’re quite large implants, they suit her overall physique better.
This is the right implant, which is a PIP 265 cc. The lot number was 32006-161. That means I think 161 implant within that batch. On the left, again, 265 cc and the batch number again was 32006, which is exactly the same batch number, but this was the 190th implant in that batch. The implants not in bad condition overall. As you saw when I removed them, they had the gel bleed. You do have quite a lot of volume loss from the top of the breast. I don’t know whether you can see that. We’re seeing that a lot with the PIPs. When they were inserted, they were fuller and firmer, but with time, the silicone bleeds out. I don’t know whether you can see you lose volume of this upper part, which a lot of our patients are complaining of. On the other side it would be a similar sort of . . . can you again see we’ve lost a bit of volume from the top, and when patients lie down, it tends to be more from the middle.
