Tummy Tuck (Abdominoplasty) & Breast Enlargement Procedure

Tummy Tuck and Breast Enlargement Operation
Hello, my name's Adrian Richards. I'm the Surgical Director of Aurora Clinics here in the United Kingdom. In this operation video, one of our series of operation videos, I'm going to be showing you an operation I did yesterday on a lady who underwent a breast enlargement and a tummy tuck procedure. She's had four children and lost a significant amount of weight and really we're going to be enhancing her bust and improving the appearance of her tummy which has got some loose tissue.
So, here we can see, this is just before surgery. Our patient's had a number of children and has lost a significant amount of volume from her upper part of her breasts, which is quite characteristic.
Here we can see actually in the operation. I've marked up the area. This is the area I'm going to plan to remove, and this is her old Caesarean scar, which I'm going to remove. Quite often the Caesarean scars are slightly different levels as in this case. That's just the tummy muscles I'm going to repair, and here's where I'm going to cut around the tummy button and bring it through on the stalk, which we're going to see in a minute.
I'm just checking here that that skin will easily remove, which it will do. I'll show you in a minute, that's where the tummy button is going to be. So, the skin is brought down, the tummy button's brought through the new skin.
So, the first stage is really just to cut around the tummy button, and I'm doing that here and then the tummy button will be on a stalk. So, you can see that, that's the stalk of the tummy button moving on either side, which I'm going to bring out through the skin as it comes down.
Here I'm just cutting through the skin on both sides. This is below the old Caesarean scar, which is going to be removed, remember.
So, a nice clean cut. This lady's got good skin. I'll make everything as neat as possible. Then, you go underneath the skin flap.
You can see here, I'll lift up the skin flap here, and you can see that's where the tummy button is. I'm just pointing to where the tummy button stalk is going to be, and I'm going to cut around that in a minute.
Here's the next stage, the skin is removed, and you can see that's the skin tissue which I've removed. You can see there, that's where the tummy button was, that's the old tummy button hole. Here's the tummy button now on its stalk with its blood supply intact.
Now this lady did have some splitting of the tummy muscles, and I'm going to repair those now. I'm just repairing this with a long lasting stitch, which you can see here, which pulls the tummy muscles together and tightens the tummy wall. So, it makes it better for doing sort of sit-ups. You can see there that I've just repaired all the way along. Tummy button's in the middle sitting there. The tummy will be tighter.
The next thing is to re-gauge where the tummy button's going to come through. That's where it wants to sit, so it's lying directly behind that. So, I'm going to make a mark where I want my new tummy button to be. That's the mark.
So, the next stage is to cut through that. As you can see here, this is where my tummy button's going to come through, remember. Then remove that tissue. You can see there's a bit of skin, a bit of fatty tissue underneath. Then my tummy button's going to come through that hole.
Here I am just after I've sewn it all in, gluing the tummy button in, and that's the tissue that's been removed.
So, you can see that's the tissue that's been removed. The tummy button has been inset.
Now, moving onto the breast, I'm going to make a five centimetre incision under the fold. This is my temporary sizer, which I'm going to place in the breast.
Here we go, it will just go in, in a minute and then I'll blow that up with the syringe to gauge the optimum volume of fluid I'd like in my implant. So, you can see the difference between the two sides here.
Then I'll insert my implant and sew everything up with absorbable stitches in this case. You can see the difference between the two sides.
This with both sides are completed, and you can see the breast implant has been completed and the tummy is nice and flat. Again, you can see from this angle, again, really good correction of the tummy and the breast.
