Breast Enlargement (or Augmentation) treatment of Tuberous Breasts with breast Implants. Mr Richards demonstrates how he releases the constriction in Tuberous or Tubular breasts and uses a silicone breast implants to correct the constriction of the breast base.

Breast Enlargement – Correction of Tuberous Breasts by Insertion of Breast Implants
Hello. My name is Adrian Richards. I’m a plastic and cosmetic surgeon and Surgical Director of Aurora Clinics. In this video, I am going to be just taking you through an operation I performed earlier on a young lady who’s got a tubular breast deformity. A tubular breast deformity is also known as a tuberous breast deformity or a constricted breast. It’s essentially characterised by a high inframammary fold and a slightly abnormal breast shape. I’m going to be showing you how I corrected this with a breast implant and also release of the constricted breast tissue internally.
Here we can see a picture of the patient from the front. You can see the nipples are at slightly different heights, and she’s also got a very short distance between the nipple and the inframammary fold. That’s what’s known as a constricted breast. You can see from the side here that short distance with a very high inframammary fold there.
I’m going to lower the inframammary fold as I’m showing here. So that’s where the old fold was, where the little lines are. I’m going to make my incision a bit lower, about seven centimetres below the nipple, and that’s going to be the position of the new fold.
Here I am just cutting through the skin, an incision about four and a half centimetres long, cutting through the fatty tissue here with my diathermy.
Now you can see this is the pocket I’ve created underneath the breast tissue. I’m going in front of the muscle in this case and you can see all clear all the way around. That’s the constriction. Can you see there with my finger I’m just pulling on the old crease? That’s tethered inside by a sort of a band which I’m just going to release now. So I sort of turn it inside out and as you can see with my cutting diathermy I just release the band. You can actually feel it releasing between your fingers and that’s going to allow the lower part of the breast to really fill out there.
So I’ve just released the constriction, and then like a normal breast augmentation I’m inserting my breast implant here. This is a temporary inflatable implant which I’m just going to use to gauge the best size for the implant. I’m just blowing that up here with my syringe. I get a fill volume of about 330 ccs in this case.
Here I am just checking that the constriction has been released. Then I insert my textured silicone implant, as I’m just doing there, through the small incision.
This is the final appearance. You’ll be able to see from the side there is still some constriction of the lower part of the breast, but I’d expect that really to fill out in a couple of months following surgery. With gravity and the implant being moved downwards, this area here is going to fill out and become more rounded. So with the release internally, with the implant you get a really good correction of the tuberous breast deformity.
