Inverted Nipple and Breast Augmentation Surgery
Hello. My name is Adrian Richards. I’m a plastic and cosmetic surgeon and the Surgical Director of Aurora Clinics based here in the United Kingdom. I’m going to be taking you, in this video, through an operation I performed earlier today in which I performed a breast augmentation and also corrected this lady’s left inverted nipple. Her right nipple was normal, but her left nipple, as we see in the photographs, has a sort of division across it like a valley and is significantly inverted. So I’ll be taking you through the augmentation procedure and also the correction of the inverted nipple.
So here we can see that this lady is quite asymmetrical with a prominent nipple on the right, but an inverted nipple on the left. There is also some breast asymmetry with a difference in the fold underneath the breast, which we can see from this three-quarter picture and also from the other side, with the right nipple being normal.
This is actually in the operation. I just performing a five centimetre incision in the inframammary fold, and this is me just dissecting through with a cutting diathermy underneath the breast tissue.
Here you can see with my lighted retractor that I’ve created the pocket all the way around there, and that’s going to be where this inflatable implant is inserted.
This is my inflatable implant, which I inflate, giving me really good control over the eventual size of the bust. And that was just temporary.
Now I’m going to put some Betadine iodine solution on the skin, put some antibiotics in the cavity here, and then insert my proper, formal prosthesis, which is a latest generation silicone prosthesis.
And you can see the right side is obviously larger than the left, and I’m really happy with that size. I think it’s a good match for her overall physique.
Now I’ve completed both sides, and now I’m just putting my stay stitch through the left nipple, the inverted nipple, just to hold it out and to put the ducts on stretch there so the ducts will be nice and tight.
And now I’m going to mark my new base of the nipple here, and then I mark 6 o’clock, 12 o’clock, 9 o’clock, and 3 o’clock positions. And between the 3 and 6 o’clock positions, I’m going to just divide the ducts. Because they’re slightly on stretch from my left hand in the stay stitch, they’re nice and tight, and with my blade here, through a very small incision, I can feel the ducts, and I’m just dividing them with a knife and then with the scissors. Here I’m just checking, as I move it side to side, I’m just checking there’s nothing holding the duct in.
And here you can see, after I divided the ducts, the nipple is out on its own.
So here we can see, this is her before the operation. This is her immediately after the operation. Obviously a larger bust and good correction of that left inverted nipple, again from the top. So you can see a good correction of both.
