Breast Reduction Surgery Revision
Hello. My name is Adrian Richards. I’m a plastic and cosmetic surgeon and the Surgical Director of Aurora Clinics, based here in the United Kingdom. In this video, I’m going to be taking you through a operation I performed earlier on today on a lady who’s previously had a bilateral breast augmentation performed 10 years ago. This used an anchor pattern, also knows as a Wise pattern reduction, but she feels as if she didn’t have enough taken off, and also she’s got quite a lot of breast asymmetry, with the right being larger than the left. In addition, the areola, which is the brown area around the nipple, is larger than she would like. So what I’m going to do is I will perform a breast reduction procedure taking more off the right, reducing the areola, lifting the nipple up a certain amount, and hopefully improving breast symmetry.
So here’s the lady before the surgery. As you can see, the nipple is larger on the right and lower on the right. You can see it from this position here, and in the next picture, you’ll be able to see the upper marking is where I’m going to move the nipple up to as you’ll see later. As you can see, on the left side, there is some scarring on that areola around the nipple, and that implies that there was some delayed healing in the original operation, possibly infection, possibly some nipple necrosis in that region, which would be superficial.
And here you can see again where I’m going to lift the nipple up to in this case.
Here you can see, that’s the pedicle I’m just showing there. And I’m going to move the nipple up to that position here, and all this lower tissue is the tissue I’m planning to remove in the operation, as you’ll see later.
Here I’m showing again from the top. The hatched area is the bit I’m going to leave, which is going to maintain the blood supply and nerve supply to the nipple. This is just a technique I use to try and make sure the nipple is as round as possible, with a nipple marker, and then just cutting around the nipple here. It’s very important to keep it really as neat as possible and as round as possible. And here I am just cutting around the area of tissue I’m going to remove in the upper part of the breast. That’s over the pedicle, so I’ll need to keep that a bit more superficial.
Here I am just taking the top layer of the skin off the pedicle, and this is a technique called deepithelialisation.
This is a technique I like to use, just to put temporary tattoos on each quadrant of the nipple. And this helps me align it up when I sew it up. It’s very important to line it up correctly to get it as round as possible.
And here I am just making my inferior cuts here, and I’m going to remove this lower tissue in the breast reduction procedure, as we’re going to see in a minute. So, I will complete that, removing that tissue, which will be submitted for histo analysis to check there aren’t any abnormalities within it.
Here you can see I’ve removed the tissue. That’s the top part around the nipple. That’s the medial part. The fatty tissue is that red, sorry, is yellow normally, as we can see, and that’s the tissue I’ve removed, which will be weighed and sent off for analysis.
And here you can see, that’s where I’m going to move the nipple up to. So it’s kept on the pedicle, which is the bridge of tissue which supplies the blood supply to it.
Here you can see I’ve done my superficial layer of stitches. These are all deep, absorbable stitches. You can see the nipple is now elevated compared to the right side. And this is with the final layer of stitches in. The next stage will be to glue that, put some Dermabond tissue glue as I’m doing here on the area just to completely seal the closure nicely and make everything really secure.
So here you can see, this is the right breast finished with the absorbable stitches. Here it is with the Micropore tape in position, which will stay in position for one week.
