Breast Uplift Surgery – Mastopexy Operation

Hello. My name is Adrian Richards. I’m a plastic and cosmetic surgeon and the Surgical Director of Aurora Clinics based here in the United Kingdom. Today, in this video, I’m going to be taking you through an operation I’ve just finished on a lady who’s lost ten stone in weight by diet alone in the last two years. She’s a relatively normal weight now and is coming forward today for a mastopexy operation, which is also known as a breast uplift procedure. In this operation, I’m going to be tightening the skin of the breast without an implant and without any reduction to improve the shape and to tighten up the skin.

Here we can see the picture of our patient from the front with a low nipple position, slightly lower on the right. The upper marks, which I put on, are where I want to implants to move to which is approximately 21 centimetres from the sternal notch. That’s the notch you can feel in the centre between your collar bones, the lower part of your throat.

Here again, you see from the side, she’s lost quite a lot of volume from the upper areas of the skin, and the nipples are lying in a low position. On the left, the nipple’s pointing down, more so on the right.

This is actually in the operation, and you can see this is the area I’m planning to excise the skin from to tighten up the breast. Now I first use a technique called tailor tacking. This allows me to check my marks. What I’m doing here is just putting some temporary stitches in where I’ve planned my marks. I’ll put a number of these in and it just gives me fine tuning. I can actually alter my skin excision slightly at this stage. This is more accurate and gives me more freedom and more accuracy actually in the operation.

I’ve done my tailor tacking; I’m happy with my marks, which you can see I’ve altered slightly there. I’ve altered them again slightly here. Here I’m just cutting through the new nipple position there where I’m going to take the top layer of skin off.

It’s very important to make the new nipple as round as possible, so I’m using this special nipple marker with ink on it to get a nice, circular new nipple position. I tend to tattoo the needle at the 12, 3, 9 and 6 o’clock positions to help me get the orientation of the nipple right where I’m sewing it in.

Here I am just cutting around the nipple. This is removing the top layer of skin, which is a technique called deepithelialisation, which is just removing the epithelium which is the top layer of skin. This preserves the dermis which has the blood supply in it. You can see here I’m removing that top layer of skin, and this is the skin that I’ve removed as you can see there.

That’s the dermis. The white tissue there is the dermis which carries the blood supply. Here I am just cutting through the lower part of the tissue. In a mastopexy, it’s important that the only bit of tissue that’s removed is the skin. I’ll move the nipple up to that position, so I’ve removed no breast tissue. It’s very important to preserve the breast tissue.

What I tend to do is free up the lower part of the breast tissue here and then sew it up here against the chest wall. This is because I want as much of the volume as possible as high as possible and as central as possible underneath the breast to give a nice, round shape. Mastopexy, as you can see, isn’t just a tightening of the skin operation. It’s actually moving the glandular tissue of the breast as well to make a nice round breast.

Here I am just freeing up around the nipple. This allows me to move the nipple up to that new position as you can see here. I’ve sewn the nipple in, and I’m allowed to do this because I’ve freed up the gland.

Here I am just sewing up the area with three layers, all absorbable stitches. You can see this is the right side completed. I’m just putting on the last layer of glue here which makes the incision nice and waterproof and allows her to shower.

Here you can see the right completed. The top again, all absorbable stitches. You can see again, from another angle here, the right and the left here you can see. Still a bit white because of the adrenaline mixture I’ve used, but you can see really much improved breast shape, contour, and nipple position.

Here’s just the final brown Micropore tape on before the bra is fitted.
