Breast Enlargement Surgery to Correct Double Bubble Effect
Hello. My name is Adrian Richards. I’m a plastic and cosmetic surgeon and the Surgical Director of Aurora Clinics based here in the United Kingdom. Today I’m going to be doing an operation on a lady who’s got a so-called double bubble effect. This is caused by the implant being placed under the muscle and causing a constriction on the lower part of the breast. What I’ll do is I’ll take you through the before pictures, then I’ll show you the actual operation and how we help this problem.
Here we can see some pictures taken directly before the operation. The folds you can see on the lower part of the breast are the position of the old inframammary folds. This has been lowered, but unfortunately the muscle is holding the implant in a high position, and this is giving an unnatural appearance to the area, known as the double bubble effect with the implant being held upwards.
Now interestingly, when she lies down it doesn’t look nearly as obvious. What I’ve done here is I’ve marked the old incision and actually have removed the scar.
Here you can just see I’m cutting down with the cutting diathermy down onto the implant. I’m just removing the implant now. The implant is intact as you’d expect it to be as six months previously.
The next stage is just to incise this area here and instil some local anaesthetic in front of the muscle. So you can see here my cannula is going in front of the muscle. And there’s the muscle. That’s behind the muscle, that’s in front of the muscle, and I’m holding the muscle in my forceps there.
The next stage is to insert an inflatable implant. This is a temporary implant in front of the muscle, and I blow it up slowly 50 millilitres at a time. This inflates the breast pocket to give me a really good analysis of the optimum fill for the breast.
Another thing I like to do is actually secure the inframammary fold. So here I am just stitching down the inframammary fold from below. This will make sure that the inframammary fold doesn’t ride up onto the lower part of the breast, and the inframammary fold lies actually in the fold and is not visible. You can see here now I’ve secured it. The inframammary fold isn’t going to go down because it’s secured. You can see there it’s nicely secured.
The next stage is to put my iodine, this is betadine I’m putting on the skin to reduce any possibility of infection.The next stage is to insert antibiotics into the cavity. These are going to lie around the implant.
I’ll then change my gloves and insert the implant into the cavity. Now you can see this implant is now going in front of the muscle. So the double bubble will be much improved because the implant will be in front of the muscle and the muscle won’t be holding it up. You only really get the double bubble with implants . . . there’s the muscle there. You can see the implant in front of the muscle. You only tend to get the double bubble with implants that are placed behind the muscle.
Here I am just closing the incision with an absorbable stitch, and I do this in three layers. You can see this is the deepest layer of absorbable stitches. I’ll do two more layers, and you can see here this is the final closure which I’m just doing with some skin glue. This just seals the wound and will allow the lady to wash and shower straight after the operation.
This is after the operation. You can see my marks at the bottom where the old fold was. It’s been nicely released.
