Hello, my name is Adrian Richards. I’m a plastic surgeon based in the United Kingdom. In this short video I’m going to be taking you through an operation in which I remove some ruptured implants which were located under the muscle in this patient.
So these are the preoperative photographs of our patient. You can see the right nipple is slightly lower than the left, and the implants are slightly higher than the nipple tissue. This is what’s known as a double-bubble effect. The reason you get this is because the implant is placed under the muscle and then the muscle is holding the implant up. In this case, the implant was placed totally under the muscle.
Here in the operation, I’m just planning my incision which I’m going to locate the muscle. You can see that I’m cutting down onto the muscle with my cutting diathermy.
Now this implant wasn’t in a partial or a dual plane position, that’s half under the muscle. It was completely under the muscle. That’s why the implant was held high. You can see there, that’s the muscle that I’m holding there. So the implant was totally covered by the muscle.
Here I am just removing the implant which was intact on that side, but on this side, not so intact.
Again, on the left, I’m just showing you the muscle there. So that low bottom muscle is called serratus anterior which was behind.
Here I am just sewing up the incision with a subcuticular suture which I’m using in this case.
The reason this implant was so high because it was totally under the muscle. That’s not the technique that I would recommend for most people. You can see here after the implant’s been removed she’s got quite a fair amount of residual breast tissue.
Here are the implants after they’ve been removed. That’s the right one, quite good condition and intact. On the left, you can see it’s quite different. Looks different, looks more wrinkly, and when I squeeze it gently there you can see. Can you see the couple of holes in the shell of the implant, which is allowing the early generation silicone to release that there. It’s really quite a good thing that this lady had her implants removed when she did.
