Inverted Nipple Correction
Hello. My name is Adrian Richards. I’m a plastic surgeon and the Surgical Director of Aurora Clinics based here in the United Kingdom. Today I’m going to be talking about an extremely common condition, which is inverted nipple correction which affects approximately 10% of the female population in the United Kingdom. This is a lady who’s got Grade 3 inversion which means that the nipples are inverted and won’t evert.
This is her after the instillation of a local anaesthetic under the nipple. The area’s been prepped and draped and the nipple is now completely numb.
The first stage is to place a traction stitch through the nipple. You can see as I lift up I’m going to be stretching the ducts. I’m marking the new base of my nipple here. I’m going to make an incision at the 12, 3, 9 and 6 o’clock position.
I made the actual incision at the base of the new nipple on the lower aspect. That’s between the 9 and 6 o’clock position on the right side. Here you can see I’m just dividing the ducts with my scalpel here. By gently dividing the ducts, you can see that the nipple is held out.
Then I’m going to place two absorbable stitches through the nipple to hold the base to prevent the area reinverting.
Dressings with this doughnut of foam, some gauze in the middle. This stays on for approximately five days and takes the pressure off the nipple. Then brown tape is put on the area, which will stay on for approximately five days.
I’m going to show you some pictures. This is a Grade 3 inversion. This is just before the procedure. And here, a bit bruised immediately after the procedure.
Here we are another Grade 2 inversion in this case. This is the picture three weeks following surgery.
