This video is of a patient who has come in for some scar revision around her areolas after having breast implants inserted at a different company. We begin the video with Mr Richards talking us through what the actual procedure is, and how he is going to do it.
The implants were inserted originally through an incision made on the lower half of her areola. Mr Richards shows his markings and has two areas drawn. He explains there is tethering in the upper area, especially when tensing. He draws on this area, explaining he will be lifting this area of skin up, and removing the layer of skin on the lower drawing. Once the layer of skin is removed, the upper portion of skin which has been elevated, will be held up, and the skin below the area which was removed will be bought in underneath the upper area and sewn together. This will not only stop the tethering but also tighten up the lower half of the breast at the same time. Mr Richards explains this is an old plastic surgery technique and the technical term for it is double breasting. After the incisions have been sewn up, a final layer of glue is applied above the absorbable stitches to secure it firmly in place.
